Congestive Heart Failure
Due to Systolic Dysfunction

Pharmacological therapy in heart failure due to systolic dysfunction

Optimal therapy to reduce mortality and morbidity in heart failure due to systolic dysfunction includes:

  • High dose ace-inhibitor or angiotensin blocker therapy. Combination therapy even better.
  • Beta-blocker therapy
  • Aldosterone blockade; eg with spirinolactone
  • Oral isosorbide dinitrate and hydralazine in African-Americans

Frusemide is used as necessary to treat congestive symptoms. Lower doses of frusemide will be required once the patient is on high dose ace-inhibitor therapy.

There is no evidence to support routine anticoagulation with warfarin in patients with heart failure.
